The Study for Development of Nursing Guideline in the Open-Heart Surgery patients


Abstract
The Purpose of this study was to provide basic data for the development of nursing guideline in open-heart surgery patients by determining the nursing needs of patients during perioperative period.
The subjects consisted of 50 adult patients who were admitted to the P University Hospital in Pusan and S General Hospital in Pucheon for open heart surgery.
The instrument for this study was developed by the researcher based on the literature and the findings from interviews.
@ES The Major findings of this study were as follows:
@EN 1. The mean scores of nursing needs were generally high in all units; postoperative thoracic surgery unit(POTU)(4.451), preoperative thoracic surgery unit(PRTU)(4.388), and postoperative intensive care unit(ICU)(4.219).
2. The leading categories of nursing needs in each unit were the informative & educational needs in PRTU(4.461), the psychological needs in ICU(4.307) and the physical needs in POTU(4.552).
3. In the physical needs, the items of high score were pain relief in both POTU(4.860) and ICU(4.800), and skillful sampling in PRTU(4.640).
4. In the environmental needs, the items of high score were cleanning and comfotable room in both PRTU(4.700) and POTU(4.540), and comfortable life in and around the bed in ICU(4.160).
5. In the informative & educational needs, the items of high score were complication and its prevention in all units; PRTU(4.980), ICU(4.570) and POTU(4.850).
6. In the psychological needs, the items of high score were the support by medical team in all units; PRTU(4.820), ICU(4.680) and POTU(4.620).
7. Structured questionnaires were designed to determine effectively in the nursing needs of open heart surgery patients.
In conclusion, the findings of this study showed that the level of nursing needs in open heart surgery patients was generally high, and the contents and levels of nursing needs were changed according to the patient's physical condition and
envieronment
in each unit.
Therefore, to provide consistent nursing intervention for the comfort and the early recovery of patients, the nurse needs to have a nursing guideline for patients whose nursing needs change constantly according to the physical & psychological
state, and
other various concerns and problems.
